PE, Athletics and Swimming Safety Plans
Starter safety plans for PE class and athletics, and for swimming. Add your gym or field, equipment and your district's heat and concussion protocols.
Starting points – Check every row against your district's policy and the venue's rules, and add what is specific to your students.

This supports, but does not replace, your district's policy. Your principal or administrator must review and approve it.
PE and athletics
Check the space and equipment before class, set boundaries, and follow your district's heat guidelines and concussion protocol: when in doubt, sit them out. Know where the AED is and who is trained to use it, and keep inhalers on the sideline.
Swimming
Choose "Swimming" from the template list. A certified lifeguard must be on duty; use a swim test to group students, and agree with the pool who supervises the water and who supervises locker rooms.
Before
- Check the gym or field and equipment
- Know the heat guidelines and concussion protocol
- Review health plans (asthma, diabetes, heart) by initials
Trip day
- Water breaks; shade in heat
- Inhalers on the sideline; AED location known
Afterward
- Report injuries
- Update the plan
Keep student information out of it
Write "Student A" or initials and say where the confidential plan is kept ("health plans are with the trip leader"). Names, diagnoses, IEP or 504 details and family phone numbers belong in your district's systems, not on a plan that gets printed and handed to chaperones. What you type here stays in this browser.

Frequently asked questions
What is a concussion protocol?
Your district's procedure for a suspected head injury: remove the student from play, notify the nurse and family, and require medical clearance before return. Follow your district's version.
When should PE move indoors for heat?
Follow your district's heat guidelines, which often use the heat index or wet bulb temperature to decide when to change or cancel outdoor activity.
Who supervises at the pool?
Certified lifeguards supervise the water; teachers stay responsible for their class, including locker rooms and transportation.
