Field Trip Permission Slip Template

Type the trip details once, then print a permission slip with a family signature section and a one-page guide for chaperones. Families fill in their part by hand.

Type straight onto the page. It saves in this browser only and is never sent anywhere. Don't enter students' personal details.

What to include on a field trip permission slip

  • Destination, date, departure and return times.
  • Transportation (school bus, charter, walking).
  • Cost, and who to contact if cost is a problem.
  • What to bring: lunch, clothing, water; what to leave at home.
  • The return-by date.
  • A signature section: student, parent or guardian, a phone number for the trip day, and a line to share health or allergy information (or contact the school nurse).

Use your district's form if it has one. Many districts have an official permission form with required wording, especially for overnight, out-of-state or water activities. This template is for trips where the district lets teachers write their own.

The chaperone guide

The second page tells chaperones their group, the meeting spot, the schedule, the rules and what to do if a student is missing or hurt. Hand it out with their group list on the day and collect the lists afterward.

Next: write the field trip safety plan and follow the planning checklist.

Keep student information out of it

Write "Student A" or initials and say where the confidential plan is kept ("health plans are with the trip leader"). Names, diagnoses, IEP or 504 details and family phone numbers belong in your district's systems, not on a plan that gets printed and handed to chaperones. What you type here stays in this browser.

Frequently asked questions

Is a permission slip required for a field trip?

Most districts require signed permission for off-campus trips, and some allow an annual form for walking trips. Check your district's policy and use its official form if there is one.

Should families write medical information on the permission slip?

Ask families to share health information with the school nurse instead, or keep any health section brief and store slips securely. Never post completed slips where others can see them.

Can I save the permission slip and reuse it?

Yes. What you type is saved in this browser, so next time you only change the trip details.
