How do I make a trip consent form or permission slip?
Use the UK trip consent form or the US permission slip template. Type the trip details onto the page, then print one for each family or download as Word.
Step by step

- Type the date, destination, times, cost, transport and what to bring.
- Leave the parent or carer sections blank for families to complete by hand.
- Print, or download as Word to send through your school’s usual system.
Questions and fixes
Should I collect medical details on the form?
Only collect what your school’s policy asks for, and store completed forms the way your school says. The template itself saves only what you type into it, in this browser.
The printout is cut off or has the wrong margins
In the print window, set the scale to “Default” or “Fit to page”, keep margins on “Default”, and check the paper size (A4 in the UK, Letter in the US). Turning on “Background graphics” keeps coloured boxes and shading.
